Punarnava is a single-herb formulation prepared from the root extract of Punarnava (Boerhaavia diffusa). Punarnava has antimicrobial and diuretic properties and is recommended in the management of urinary tract infections (UTIs) and associated symptoms. Punarnava contains glycoproteins that have antibacterial activity against RNA bacteriophages. Punarnava possesses diuretic action, which can be attributed to its constituent ẞ-ecdysone, a xanthone, which inhibits the kidney's succinic dehydrogenase. Punarnava brings about anti-inflammatory action by inhibiting the release of proinflammatory mediators and prostaglandin synthesis.

Punarnava possesses a dose-dependent anti-inflammatory activity. It stabilizes the cell membrane, thereby inhibiting cell lysis and the release of proinflammatory mediators. During inflammation, lysosomal hydrolytic enzymes are released, and this damages the surrounding organelles and tissues, resulting in a variety of disorders. Inflammation is mediated through the release of mediators including vasoactive substances such as histamine, bradykinin, serotonin, and prostaglandins. These chemicals promote the accumulation of fluid in the tissues and thus edema. Therefore, the mode of action of Punarnava extract is possibly through the prevention of the release of proinflammatory mediators and inhibition of prostaglandin synthesis, which further results in the inhibition of inflammation.
3. Diuretic activity
Punarnava has potent diuretic activity. The diuretic activity of Punarnava can be attributed to the potassium and B-ecdysone content in it. It stimulates d-amino acid oxidase and inhibits succinic dehydrogenase in the kidney, thereby resulting in diuretic action.
